Black smoke billowed in the air, as soldiers carried the injured on stretchers to waiting ambulances.<br /><br />Military spokesman Sagom Tamboen said the plane was transporting troops and their families, including at least 10 children, when it tumbled from the sky near an air force base in East Java province. It slammed into several houses in Geplak village, killing three on the ground, and then skidded into a rice field.<br /><br />It was not clear what caused the crash.<br /><br />But air force spokesman Bambang Sulistyo said the plane was nearly 30 years old and several witnesses described seeing its right wing fall off while it was still in the air.<br /><br />"I heard at least two big explosions and saw flashes of fire inside the plane," said Lamidi, a 41-year-old peasant who was working in his rice field. "The wing snapped off and fell to the ground."<br /><br />The plane's charred tail and several chunks of its wing and body were scattered across the paddy.<br /><br />Tamboen said the plane was carrying 109 passengers and crew when it went down 325 miles (520 kilometers) east of the capital, Jakarta. The death toll climbed to 93, he said. At least 15 people survived, Sulistyo said, many with bad burns.<br /><br />The country's air force has long complained of being underfunded and handicapped by a recently lifted U.S. ban on weapons sales. It has suffered a series of accidents, including a Fokker 27 plane that crashed into an airport hangar last month, killing all 24 onboard.<br /><br />The C-130s are considered the backbone of the transport wing. The air force has operated the giant planes since the early 1960s, when it received a batch of 10 from the United States in exchange for the release of a CIA bomber pilot shot down in 1958 while supporting an anti-government mutiny.<br /><br />About 40 more were inducted into the air force inventory over the next 20 years. Many were secondhand and provided by Washington before the Clinton administration introduced sanctions on military deliveries over bloodshed in East Timor in 1999.<br /><br />The air force complained that many of the planes quickly became unserviceable because of the lack of spare parts. Though the embargo was lifted by the Bush administration, the air worthiness of many is still in question.<br /><br />Tamboen promised a full investigation of all military aircraft.<br /><br />There have also been a series of commercial airline crashes in recent years, killing more than 120 people. Investigators believe wind shear, or a sudden gust of wind, may have been a factor.<br /><br />Questions were also being raised about the safety of the MD-11, a wide-body airliner built by McDonnell Douglas and based on the DC-10.<br /><br />The flight from FedEx's hub in Guangzhou, China, appeared to bounce after its initial touch down, and then skipped along the main runway at Narita Airport before flipping over and coming to a fiery halt, footage from airport security cameras showed.<br /><br />Firefighters and rescuers immediately swarmed the MD-11 plane but the pilot and copilot — Kevin Kyle Mosley, 54, and Anthony Stephen Pino, 49 — were killed. Mosley lived in Hillsboro, Oregon, while Pino was from San Antonio, Texas, according to online records at the Federal Aviation Administration.<br /><br />They were the only two people aboard.<br /><br />Investigators said the accident may have been caused by low-level turbulence or "wind shear," sudden gusts that can lift or smash an aircraft into the ground during landing, said Kazuhito Tanakajima, an aviation safety official at the Transport Ministry.<br /><br />Unusually strong winds of up to about 47 miles per hour (76 kilometers per hour) were blowing through Narita City on Monday morning around the time of the crash, according to the Japan Meteorological Agency.<br /><br />But Tanakajima said the wind speed at the time of the accident was not enough to be considered dangerous, unless wind shear was involved. He said there was headwind of about 45 miles per hour (72 kilometers per hour), and a crosswind of about 7 miles per hour.<br /><br />Strong winds and turbulence have caused other recent incidents at the airport.<br /><br />Last month, a flight from the Philippines was jolted by severe turbulence as it circled prior to landing, injuring 50 passengers and crew members.<br /><br />The MD-11 has been involved in accidents in which it flipped while landing, and pilots have complained about the aircraft in the past. The plane is no longer used by carriers for passenger travel but is widely employed for moving cargo.<br /><br />In 1999, an MD-11 flipped over and burst into flames, killing three people during a crash landing in a storm in Hong Kong. And in 1997 one of the planes landed hard, flipped and caught fire while landing in Newark, N.J.<br /><br />Tomoki Kuwano, a former Japan Airlines pilot and aviation expert, said that although wind shear could not be ruled out, the MD-11 has a tendency to be unstable during landing.<br /><br />"In the past, the MD-11 has a record of landing failure," he said. "And when that happens it often flips over."<br /><br />FedEx said it was investigating the cause of the accident.<br /><br />"We will continue to work closely with the applicable authorities as we seek to determine the cause for this tragic incident," it said in a statement.<br /><br />The plane smashed into the longer of Narita's two runways, which remained closed Monday with all incoming flights diverted, said airport spokeswoman Misuho Fukuda.<br /><br />Parts of the wreckage were still burning hours after the crash, forcing the cancellation of dozens of flights. At least 10,000 passengers were affected, according to airlines contacted by The Associated Press.<br /><br />Japanese media reported that Monday's was the first fatal crash at Narita Airport, a major international hub located about 35 miles (60 kilometers) east of central Tokyo. All passengers were safe but 30 percent of them suffered shock.<br /><br />Herry said although nobody was killed in the two incidents the authorities considered it important to check all MD-90 planes because they had often been involved in accidents.<br /><br />"The checks are to be done as an anticipatory effort to prevent the recurrence of incidents. It is also to be done to make sure that all of the planes are still airworthy. The decision on whether or not the planes are airworthy will be taken fairly and wisely after thorough investigations," he said.<br /><br />He said the pilots that had flown the ill-fated planes meanwhile had been banned from flying any plane for a month pending the results of the investigation on the incidents by the National Committee for Transportation Safety (KNKT).<br /><br />"If the results show human error they will be given a sanction," he said.<br /><br />He said plane accidents could be caused by various factors but the main three of them were human, weather and technical factors. In view of that he said a thorough study had to be done to find out the real cause of the two incidents and this was the authority of the KNKT.<br /><br />"Study was also needed to see if the planes have routinely been checked in line with aviation rules and the law," he said.<br /><br />Herry said accidents could happen on any plane including the new one accidents. He said the government could not as yet tell if the MD-90 type could still be operated or not.<br /><br />According to the ministry's data only Lion Air operated MD-90s totalling five units. The airline has recently suffered safety problems and financial difficulty.<br /><br />Formed in 2003, Adam Air hit financial crisis on Monday when two major shareholders, investment companies who control 50% of the airline, anounced they were pulling out and selling all shares back to the original owners, who still control the other half. At the airline's height, it operated 22 of these, serving domestic routes accross the Indonesian archipelago and daily international flights to Malaysia and Singapore.<br /><br />Adam Air also had a serious incident in early 2006, when all navigational systems on a B737 failed, causing the plane to inadvertantly enter an accident blackspot. The aircraft was lost for several hours before performing an emergency landing hundreds of miles from its intended destination.<br /><br />On March 10, another Adam Air B737 overshot the runway during landing at Batam island's airport. Since then, Adam Air has been issued two deadlines in rapid succession by the authorities - one to solve safety issues, and one to prove its financial viability. They have cut routes from 52 to 12, and defaulted on all payments on their aircraft to the leasing companies, who have seized back 12 of them.<br /><br />Gustiono Kustianto, director of one of the two leasing companies, said the carrier's "life expectancy is less than a month," and that it owes US$14 million to leasing companies compared to $4.8 million of free capital.<br /><br />Aviation director general Budhi Mulyawan Suyitno said that Adam Air had been grounded due to inadequate Maintenance, Repair and Overhaul (MRO) standards and poor pilot training. He said that investigations into last week's crash, which injured five people and severely damaged the aircraft, had revealed "the pilot had no idea what to do in an emergency situation." The airline will spend three months grounded, after which it will be shut down entirely if it does not show it has improved.<br /><br />"The Transport Ministry has decided to revoke Adam Air's operational specification, effective 12am Wednesday (4am AEDT), said Suyitno. "With this, Adam Air is banned from operating its aircraft. All of its planes must be automatically grounded." He commented that a quaterly evaluation had uncovered "violations that could put passengers' safety at risk".<br /><br />The news coincides with a two-day ultimatum from Adam Air's insurance provider. Failure to pay the insurance premiums would also have resulted in grounding.<br /><br />Founder Adam Suherman - whose family owns half the airline alongside Sandra Ang - commented today that the situation with the shareholders had demoralised pilots. "That in turn would affect their performance. It is very dangerous." He also said that he was not surprised Adam Air had been grounded. The accident occurred as Garuda Flight GA 200, with seven crew members and 133 passengers on board, arrived at Yogyakarta on a scheduled flight from Jakarta. Twenty-one people lost their lives after the aircraft overshot the runway at Yogyakarta, broke through a fence, crossed a road, and came to rest in a rice paddy where it caught fire.<br /><br />In October of 2007, Indonesia's National Transportation Safety Committee (NTSC) issued a report on the accident, as well as an English-language media release about their findings. The media release stated that the NTSC's main finding was that "...the flight crew’s compliance with procedures was not at a level to ensure the safe operation of the aircraft."<br /><br />More specifically, the NTSC media release said this of Capt. Marwoto Komar:<br /><br />The aircraft was flown at an excessive airspeed and steep flight path angle during the approach and landing, resulting in an unstabilized approach. The PIC did not follow company procedures that required him to fly a stabilized approach, and he did not abort the landing and go around when the approach was not stabilized. His attention was fixated or channelized on landing the aircraft on the runway and he either did not hear, or disregarded the [Ground Proximity Warning System (GPWS)] alerts and warnings and calls from the copilot to go around.<br />Many in the international aviation community have expressed concern that the NTSC investigation fell short by not investigating more fully the reasons behind the actions of the captain, as well as his co-pilot, Gagam Jahman Rochman. The latter was accused of not following company procedures to "take control of the aircraft from the PIC when he saw that the pilot in command repeatedly ignored the GPWS alerts and warnings."<br /><br />Upon hearing of Marwoto Komar's arrest, the Federation of Indonesian Pilots (FPI) staged a protest rally at the House of Representatives in Jakarta. The pilots, led by FPI president Manotar Napitupulu, told members of the House Commission in charge of transportation that this "criminalization of a pilot" could eventually affect flight safety. Their view is shared by many others.<br /><br />The International Federation of Air Line Pilots' Associations (IFALPA), an advocacy group representing more than 100,000 pilots in more than 95 countries worldwide, issued a statement expressing concern over the arrest of Marwoto Komar, citing concerns that echo those of the FPI. Here is an excerpt from the IFALPA statement:<br />IFALPA believes that the circumstances of the accident as set forth in the final report of the Indonesian investigation authority leaves many serious questions concerning the crew actions prior to the accident. Central to these concerns are the underlying reasons for the reported behavior of Captain Marwoto Komar. Experienced pilots have considerable difficulty in attempting to explain what is reported in the context of normal airline operations.<br /><br />The Federation believes that the explanations proffered by the report do not square with the collective experience of our members. The Federation has continually maintained that the report, while final, is in fact incomplete and that additional investigation into the underlying pathology of the crew actions is required to make certain that the factors contributing to the observed actions are fully identified. Unless this is done, there is little possibility that aviation safety in the area of<br />crew performance can be improved by the lessons of this accident. Clearly, a criminal prosecution at this time may well foreclose further investigation for safety purposes.<br /><br />IFALPA is firmly of the belief that the criminalisation of individuals involved in accidents and incidents does little to improve air safety. Furthermore, IFALPA strongly insists that the principles recommended in Attachment E of ICAO Annex 13, which hold that there should be no criminal liability without intent to do harm, be the standard to which the crew is held. The Federation demands that any Indonesian criminal proceeding respect both these principles and the concept of due process.<br /><br />The Federation expects that Captain Marwoto Komar will be released without the need to post a monetary bond as he has agreed to fully cooperate with the police investigation and clearly poses no danger to society. He remains a professional<br />who was involved in an unfortunate tragedy.<br /><br />IFALPA will continue to closely monitor the criminal proceedings with the aim of ensuring that the judicial process in Indonesia is fundamentally fair and impartial for all crewmembers.<br />An article on the Australian news website News.co.au reports that Marwoto Komar's attorneys had requested that he be released from detention on bail, but that the request had been denied. The B-737-400 aircraft with 171 passengers slid 70 meters toward one side of runway. Three people were injured with one remaining unconscious. The victims were taken to the hospital of Otorita Batam. <br /><br />Danke Drajat, director of Adam Air communication department, said there were no severely injured passengers in this incident. “Only several are shocked”, he said. He confirmed all passengers have been evacuated. Adam Air, he said, has submitted this incident to the National Flights Safety Commission.<br /><br />Razali Akbar, Head of Hang Nadim Airport, said that before the skid, the aircraft had touched the runway but it suddenly flew back. The landing then had to be done again. On the second landing , the aircraft looked unstable. After it landed, the aircraft turned to the right and went off the runway. “It is not clear why it suddenly skidded”, Rajazali said.<br /><br />Razali said there were no sparks when the aircraft skidded. After it stopped, the front wheel was left dangling while the back wheel was down on the ground. Both left and right wings were broken. “It was raining when the aircraft landed”, he said. <br /><br />Razali also confirmed that the runway condition was proper for take-off and landing because the viewing distance was 3,000 meters more out of a total of 4,025 meters . The runway and all navigation devices were functioning well. “This condition is adequate for the landing of a B737-400”, he said.<br /><br />In response to this incident, the provincial police of Kepulauan Riau, held Sugiarto, pilot of Adam Air, while waiting for the KNKT team from Jakarta. Police also secured the plane to prevent any unexpected accident.<br /><br />For two hours, the airport of Hang Nadim was closed for taking-off and landing. Hence, there were delays for the flight to Natuna (Merpati Airlines) and Jakarta (Air Asia and Lion Air). <br /><br />The landing of several airlines was changed to another airport. Batavia airline from Jakarta, landed in Pekanbaru. It is starting to fly like a bamboo ship," said one of the two pilots before the Boeing 737 crashed, according to comments captured by the cockpit voice recorder. "This is crazy!"<br /><br />Last week, the government revoked low-cost carrier Adam Air's operating license because of its poor safety record.<br /><br />The National Transportation Safety Committee said 154 recurring defects in the plane's navigation system were reported in the months leading up to New Year's Day crash, and that the carrier failed to properly address those reports or train pilots to deal with them.<br /><br />The plane was flying from the main island of Java to an airport in the east of Indonesia when it spiraled from the sky at a height of 33,000 feet. In the course of trying to fix the problem, the jetliner's autopilot disengaged, causing the plane to bank to the right.<br /><br />The pilots were apparently unaware they were now flying the plane and ignored "a number of initial alerts, warnings and changes to displays" indicating the jetliner's increasingly critical situation, the report said.<br /><br />"The pilots did not detect and appropriately arrest the descent soon enough to prevent loss of control," it said, adding that they apparently had no training on what to do if the Inertial Reference System failed, nor if the autopilot unexpectedly disengaged.<br /><br />They made several wrong decisions in the seconds after the autopilot was turned off, the report said.<br /><br />The accident was one of a spate in Indonesia in recent years, including one involving the national carrier Garuda that killed 21, leading the EU to ban all Indonesian airlines and the United States to warn that they did not meet international standards.<br /><br />Adam Air was one of dozens of new airlines to emerge in Indonesia after it deregulated its aviation industry in the 1990s. Changes include a new, two crew digital flightdeck with six large CRT displays, an increased span wing with winglets (the -400 was the first airliner to introduce winglets), new engines, recontoured wing/fuselage fairing, a new interior, lower basic but increased max takeoff weights, and greater range.<br />Apart from the basic passenger 747-400 model, a number of variants have been offered including the winglet-less 747-400 Domestic optimised for Japanese short haul domestic sectors, the 747-400M Combi passenger/freight model, and the 747-400F Freighter (which combines the 747-200F's fuselage with the -400's wing). The latest model is the 747-400ER, which was launched on November 28, 2000 when Qantas placed an order for 6. The -400ER has the same size as the -400, but has more range or payload capability. The MTOW was increased by 15,870kg (35,000lb) to 412,770kg (910,000lb), giving a further range of 805km (435nm) or a 6800kg (15,000lb) greater payload. A cargo version, the 747-400ERF, followed the standard -400ER, and was launched April 30, 2001 on an order by leasing company ILFC for 5. The first -400ERF is the 1315th 747 built.
